Enlarge your penis, or enjoy the one you have

We all get the emails for penis enlargement, even those of us without penises. I’ve never looked into them before, I just assumed they were all ridiculous, and perhaps even harmful.

The most common in the ads are pills:

Maybe a few of these “miracle” supplements bring, like, a milliliter more blood to the target area, but you could accomplish that yourself by being in a little better shape. There are many supplements claiming to make you 30% larger or even more, but the only thing getting huge is the profits of the companies that sell them.

Not to mention that you have no idea what might be in them.

Penis pump:

A penis pump is a plastic tube with a rubber seal on the end. You insert your manhood, and the rubber creates a vacuum seal around the base of your member. When you squeeze the trigger, it sucks the air out of the tube and pulls more blood into your penis, which pumps it up….

Oh, and if you pump it up too much, you can end up with a freakish doughnut ring thing around your dick, like a tire tube that never, ever goes away.

Pumps have been around for over a century, which suggests that penis size anxieties predate ubiquitous porn.

Some have claims of increasing length by

Phalloplasty:

First, the suspensory ligament that attaches the penis to the pubic bone is severed. Then the penis is moved and reattached a little farther out from the body for a slight gain in length. Finally, the penis is made thicker by suctioning fat from a fleshy part of the body and injecting that fat into the penis.

Jelqing:

It is performed by repeatedly stroking the penis in a squeezing motion from the base of the shaft to the corona of the glans…

Jelqing could potentially result in blood vessel tearing, scar formation, pain, disfigurement, and desensitization of the penis

Stretching:

This technique involves placing a weight or a small extending frame, sometimes called a traction device, on the flaccid penis to stretch it. Traction devices may increase length by a centimetre or so over time. Traction devices sometimes used prior to penile prosthesis implantation.

Clamping:

The goal of clamping is to increase the size of the penis using a constricting device, such as a shoe string, cable clamp, or a tight cock ring to restrict blood flow out of the organ. The device is firmly attached at the base of the erect penis, and the man engages in “edging” (extended masturbation). A metal cock ring is dangerous because blood trapped in the penis can make removal of the ring impossible without emergency intervention such as amputation or sawing the ring off.

Penis girth surgery:

Some surgical techniques to increase penis girth involve injecting fat taken from another part of the body into the penis. Some studies have reported increases in circumference of 1.4-4cm. However, studies that followed men up over a longer time suggest disappointing results, with complications including disfigurement, scarring, lumpiness and infection. Another method, still in its experimental stage, involves pulling back the skin of the penis to wrap a tube-like biodegradable frame filled with tissue cells around the shaft.

Injecting Vaseline:

Penile girth enhancement by the injection of Vaseline is an existing practice. Many cases develop severe complications that need surgery.

This practice has led to an article in the Journal of Sexual Medicine on surgical techniques:

To  report on the reconstructive surgical solutions of the complications of Vaseline self-injection and the outcomes. To develop a modification of a one-step reconstruction method involving the use of pedicled scrotal flaps. To report on the reconstructive surgical solutions of the complications of Vaseline self-injection and the outcomes. Injecting silicon and/or mineral oils:

Abstract
Background: Complications resulting from enlargement of the

penis by applications of unknown types of silicone and mineral oils are well described. Surgical removal of the tissue altered by inflammation leads to the development of defects of various sizes, often circular from the glans penis to the scrotum. The options of subsequent surgical treatment described in literature are not very extensive. Most defects are managed with skin grafting, rarely V-Y advancement or bilateral scrotal flaps. Methods: We present a 36-year-old patient after application of unknown silicone material into the penis for cosmetic enlargement. After the application developed severe inflammation with ulceration and necrosis around the penis. Conservative treatment was not effective, therefore, the infiltrated skin with subcutaneous tissue of the entire penis was surgically removed. The resulting defect was covered by implantation of the penis under the skin of the scrotum.Result: There were no complications in the postoperative course, pain that was present before the surgery immediately subsided. Skin suture healed completely within 14 days. Three months after the surgery the patient returned to normal sexual life. Conclusions: Implantation of the penis under the skin of the scrotum is a fast, safe and effective method that can treat most of the circular skin defects of the penis. Scrotal skin is thin, soft, elastic and creates abundant and good cover around the entire penis.

Injecting cod-liver oil:

Various amounts of cod liver oil were injected in the subcutaneous area of the penis of 8 low socioeconomic class patients by a nonmedical person. They presented by various complications ranging from paraphimosis up to abscess formation and necrosis of penile skin. All the patients underwent emergency initial surgical intervention ranging from dorsal preputial slit to skin debridement. Definitive surgical treatment was carried out using local penile flap and V–Y plasty.

The severity of complications was correlated to the amount of oil injected and the time interval between injections and presentation. All the 8 patients were cured after different staged surgical procedures. The postoperative course was uneventful in 6 patients, and 2 patients suffered from wound infection after the secondary treatment. All patients had acceptable cosmetic and functional outcome, and were satisfied regarding the length of the penis. None reported erectile dysfunction.

There is a condition known as micropenis that has specific diagnostic criteria.

Micropenis is a medical diagnosis based on correct measurement of length. If stretched penile length is below the value corresponding to – 2.5 standard deviation of the mean in a patient with normal internal and external male genitalia, a diagnosis of micropenis is considered. Micropenis can be caused by a variety of factors including structural or hormonal defects of the hypothalamic-pituitary-gonadal axis. It can also be a component of a number of congenital syndromes. For the etiological evaluation, endocrinologic tests are important.

Treatments include hormones or possibly surgery by a pediatric urologist.  Surgical techniques used are based upon those developed for gender reassignment (female to male).  Some physicians recommend infant gender reassignment surgery however, there is a movement away from this as a less binary view of human gender becomes more recognized.

Insecurity related to penis size is very common in men in many cultures, as size can be tied to a cultural belief in masculinity. In extreme cases this can be related to anxiety disorders, or in more extreme cases, considered a subset of Body Dysmorphic Disorder (BDD). Treatment should be primarily within the mental health community where techniques such as Cognitive Behaviour Therapy and medications can be effective. BDD, based on any physical attribute, is not a trivial condition as it can severely impair social a involvement and personal relationships.

Thus, is you are concerned with penile size, it is wise to visit a physician to determine if your penis is within the normal range. At that time, and depending upon the physician’s advice, follow up with a mental health professional, a urologist, a plastic surgeon, or other specialist might be indicated. If you are concerned with your ability to please your sexual partners, visiting a sex therapist might assist in alleviating your concerns.

The products advertised on email, on the web, or in print are all based upon insecurities rather than science. Don’t waste your money or your time. The most effective means of penis enlargement is temporary engorgement with blood and this can readily be accomplished by yourself or with partners. Even if the effects aren’t permanent, they are a lot more pleasurable than any of the alternatives.
